Some one from the dealership is signing as both buyer and seller using a power of attorney ( form 82053) when disclosing mileage on a sale.

You will need a new power of attorney appointing a third party signed then a letterhead affidavit is needed to state who the correct power of attorney is and they will need to sign the title and print their name.
If there is no one else to sign on behalf of the dealer that person needs to be the only sole proprietor of the dealership which needs to be verified in FRVIS. If in fact they are the only person we will need a letterhead affidavit stating they are the only employee and sole proprietor of the dealership.

An odometer disclosure and letterhead affidavit is needed to state what the correct mileage and/ or mileage date is.
Sale did not go thru but the title was already filled out.
A letterhead affidavit is required from the selling dealer stating that person never took possession of the vehicle and to void that reassignment.
A Customer lost their plate and needs to be replaced.
Form 83146 needs to be completed and signed by the customer to be able to replace that plate and use any credit available.
